Dr. Hogan and Dr. Solomon and his staff wish to welcome you to our office. Your health and well-being are our primary concern. We hope that the information provided here answers many of the questions you might have about our services, policies, and procedures. Doctor/Patient Relations
The relationship between the doctor and his patient is confidential. It is a relationship based on mutual trust and understanding. Your medical/dental records are absolutely private. No information about your condition will be given to employers, friends, or relatives without your permission (except if required by a court of law). We want you to fully understand your treatment. If you do not understand something, please feel free to ask questions. Also, your suggestions or complaints are important to us because our doctor and staff are interested in ways that we might improve our services.

Release of Information
You may request that we release your medical/dental records to a third party, such as an insurance provider or attorney. Before we release these records, you will have to sign a form authorizing us to release this confidential information.

Phone Calls
Telephone calls regarding your appointment, condition, medication, or treatment are taken from 8:00am to 5:00pm Monday through Friday. At other times, an answering service is available. The answering service will refer your call to the doctor or the doctor on call.

Clinical Hours
Clinical hours are 8:00am to 5:00pm Monday, Tuesday, Thursday, and Friday.  The front office staff is present on Wednesday's for scheduling appointments and questions.

Appointments
Patients are seen by appointment. Please call in advance so that we may reserve a time for you. We ask that if you cannot make your appointment with us, please call at least 24 hours in advance. We make every effort to be on time for our patients, and ask that you extend the same courtesy to us. Please understand that not all patients require the same amount of time with the doctor and that emergencies do occur, so some delays are unavoidable. We will do our best to keep you informed of delays. Your patience in these situations will be greatly appreciated.

Emergency Care
We reserve a limited amount of time each day so that emergency patients may be scheduled quickly. If you have a problem requiring emergency treatment, please call us as early in the day as possible. Should an emergency occur after the office hours, the doctor can be reached by calling our answering service.

Registration
On your first visit to our office, you will be asked for basic information to establish your dental/medical record and business account. Please bring your current insurance information at that time and notify our office of any changes in name, address, phone number, or insurance as soon as any change occurs.

Payment
In an effort to hold the line on dental/medical costs while maintaining a superior level of professional care we offer the following payment options:
1. Payment in full, or payment of an estimated amount of the portion your insurance will not cover on the day service is provided.
2. Payment of balance in full upon receipt of your statement.
3. Use of your VISA, DISCOVER, or MASTERCARD.
If other arrangements are needed please talk to our business office staff prior to receiving services.

Insurance
The Oral Surgery and Dental Implant Center participates in a variety of health care insurance programs which aid in the payment of your medical/dental costs. Should there be problems with an insurance claim, we suggest that you first direct your questions to your insurance carrier. Our business office will be pleased to help you if they can be of any assistance in resolving a problem.

Worker’s Comp
As a courtesy to our patients, our Business Office will file Worker’s Compensation claims. However, if the claim is denied, unsettled or not paid within 60 days, we will request that you file a personal health insurance claim or pay the charges in full. If the situation becomes a legal matter, you are still ultimately responsible for the payment of charges.
