Please bring your medical and dental insurance cards and a photo identification (for adults) to your first office visit so that we can expedite reimbursement.

We do submit claims to private insurance companies and will do our best to determine what expenses are covered.  

We accept cash, checks, MC/Visa/Discover and Care Credit.
